End User License Agreement (EULA)

Effective Date: 8/15/2025

This End User License Agreement (“Agreement”) is a legal agreement between you (“User”) and Triad Diagnostic Solutions (“Company”) regarding your access to and use of the CHUCK Integration (“Application”). By using the Application, you agree to be bound by this Agreement. If you do not agree, do not use the Application.

1. License Grant

The Company grants you a limited, non-exclusive, non-transferable, revocable license to access and use the Application for your personal or internal business purposes, subject to this Agreement.

2. Restrictions

You agree not to:

  • Copy, modify, or distribute the Application.

  • Reverse engineer, decompile, or disassemble the Application.

  • Share access credentials with unauthorized parties.

  • Use the Application for unlawful purposes.

3. Ownership

The Application and all related intellectual property rights remain the exclusive property of the Company.

4. Termination

The Company may suspend or terminate your access at any time, with or without cause. Upon termination, you must cease all use of the Application.

5. Disclaimer of Warranties

The Application is provided “as is” without warranties of any kind, whether express or implied.

6. Limitation of Liability

To the maximum extent permitted by law, the Company is not liable for any indirect, incidental, or consequential damages arising from your use of the Application.

7. Governing Law

This Agreement is governed by the laws of Indiana, without regard to its conflict of law provisions.
